For families or patients interested in home care, contact the Intake Department at 908-788-2562 to discuss your home care needs. The Intake Department will then contact your doctor to obtain necessary paperwork and orders for home health. You will need to have a recent physician visit and your doctor would need to agree to sign the home health orders. The Intake Office is open Monday through Friday from 8:00am-4:30pm. If it is after hours, you may contact the main Home Health Line at 908-788-6138 to speak with an on call nurse.

Medical Professionals Only:

If you are interested in having your patient seen by our Home Health Team, you can fax a copy of the last office visit, demographics, and insurance to the Intake Department, 908-788-6681.
If your patient is Medicare primary, regulations state that the patient must be seen by a doctor within 90 days prior or 30 days after the start of home health services that pertain to the reason the patient needs home care services. Medicare requires the physician to complete a face-to-face encounter with documentation of the reason why the patient needs home care and what clinical findings and functional status that causes patient to be home bound.
