Your primary care physician is specifically trained to be your first point of entry into the healthcare system and will be your advocate in coordinating your ongoing care and making referrals to specialists.
Your primary care physician will monitor your blood pressure, cholesterol and glucose levels and can help you reach or maintain a healthy weight. If you are at risk for heart and vascular disease or have any changes in your health that indicate increased risk, your primary care physician may determine that specialized care is needed. Your physician can then work closely with our heart and vascular team to coordinate the best plan of action.
