Hunterdon Healthcare to Add Additional Pharmacists to Primary Care Practices

Medications are increasingly important in managing diseases. Advances in drug treatment offer life-changing and even life-saving options. “With more complex drug treatments comes a greater need for special focus on medication management in combination with lifestyle. Pharmacists are well positioned to play a vital role and have the necessary skills to help providers improve therapeutic outcomes,” explained Navin Philips, Pharm.D, B.S., Director of Pharmaceutical Services, Hunterdon Medical Center. Horizon Blue Cross Blue Shield has funded resources for Hunterdon Healthcare Partners, a partnership between Hunterdon Healthcare System, and Hunterdon Physicians, to add a clinical pharmacist to the primary care teams. The pharmacist will work with data from Horizon as well as from Hunterdon Healthcare, and work with the primary care offices to help complex patients with their medication treatments. The goal is simpler, safer, and optimally effective medications at the lowest cost.
The Hunterdon Medical Center Pharmacy Department has a diverse team of highly skilled pharmacists. These clinical pharmacists have a doctoral degree in pharmacy and are residency trained. They are specially trained to work with patients and care teams, focusing on patients taking multiple medications, drug interactions, optimal dosing, safety, and more. Pharmacists help patients understand how to get the best from their medications, and how to manage the ever-increasing cost of drug treatments.
Hunterdon Healthcare, clinical pharmacists work with teams and patients every day.
