The Hunterdon Center for Healthy Aging
Our goal is to optimize the quality of life for older adults, their caregivers and family members. Our Programs Include:
Geriatric Assessment is a comprehensive evaluation of the older adult by a team of health care professionals specializing in geriatrics (care of the older adult).
Since your health represents a combination of many complex factors, your Geriatric Assessment will gather information about health habits, lifestyle and living arrangements, home safety, and other aspects of your life. Social workers or nurses from the Geriatric Assessment team will examine these issues with you.
A geriatrician (a physician specializing in the care of the elderly) or geriatric nurse practitioner will conduct your physical examination, which includes a complete health history for you and your family.
Following your physical examination and information gathering, the physician may order additional tests, blood work or x-rays. These may be conducted immediately following your examination.
In some circumstances, a nurse or social worker may visit you at home to initiate your Geriatric Assessment, allowing them to examine your special needs in the context of your home setting.
Defensive driving course for 50+ drivers. Given once a month,co-sponsored with the AARP.
“The Aging Experience”
Sensitivity training for the professional and non-professional caregiver. Participants learn and “feel” what it is like to be aged.
An annual educational program for seniors, usually given in the spring.

Why Lifeline from Hunterdon Lifeline is the #1 choice
When it comes to experience, capabilities and that special caring touch, no other service lives up to the reputation of genuine Lifeline. More caregivers trust Lifeline to look after their loved ones. Today, Lifeline helps over 500,000 seniors or disabled people to live with greater independence and dignity in their own homes. The cost of all this protection? Just $44 a month and a $65 activation fee, with no hidden costs, no long-term contracts, and you may discontinue the service at any time. Yes, you can save a few dollars every month on cheaper medical alarm services, but we don’t believe you’ll find any that can deliver this invaluable service as well as Lifeline.
Lifeline has the largest and most experienced group of response professionals. With the simple press of a button, at any time of the day or night, you or your loved one is quickly connected with a highly trained Lifeline Personal Response Associate. The Associate quickly makes voice contact, reviews the individual’s personal and medical profile, assesses the situation and then notifies the appropriate support and medical response team as required. Lifeline’s Response Centers, with a staff of over 250 people, are simply the most capable and reliable facilities of their kind.
Hospice Care: It’s About How You Live
Hospice is not a place but a philosophy of care that focuses on palliative/comfort care, not cure and on the quality of life, not on the duration. Hospice is for people with a progressive, life limiting disease and their families/caregivers. Care and supportive services are brought to the patient, whether they live in a personal residence, assisted living or nursing home setting, and help to make each day the best that it can be. By bringing the care to you, patients and families can focus on being together.
Under the direction of a physician, hospice uses advanced methods of palliative care- pain and symptom management – that enables the patient to live as comfortably as possible.
