Please present your insurance card at the beginning of each appointment and let us know if there have been any changes in your coverage. We will help you with your plan as it relates to benefits, referrals, and billing; but ultimately it is your responsibility to understand your insurance.

Please remember to bring your insurance card with you each time you visit our office.  It is also important to have a good understanding of your healthcare benefits.  Please call your managed healthcare provider directly with any questions.
