Charlestown Family Health Center offers the following services:

- Lab services and clinical testing:
- Test results are usually available in one week.
- We will contact you as soon as we have received and reviewed the results; please wait for us to contact you.
- Be aware that a blood test and other diagnostic exams require a doctor’s order.
- Records transfer, should you find it necessary to transfer them to another physician:
- We ask that you complete a medical records release authorization form.
- An outside vendor provides the photocopying services.
- Nurse visits are available …

- for routine labwork and testing
- only at the order of one of our physicians
- Prescription refills:
- We will gladly refill your approved prescription over the phone. Call our pharmacy lines at:

- pharmacy
- phone number where you can be reached
- Approved refills are sent to the appropriate pharmacy within 24 hours.
- We will contact you if we are unable to fill your prescriptions.
