How to Make an Appointment
1. Most insurance plans cover diabetes management education and diabetes nutritional counseling. Check with your insurance company for what your specific plan covers. Often nutrition counseling for weight management is not covered.
Medicare covers 10 hours of initial diabetes management education in a 12 month period plus 2 hours follow-up annually. You would be responsible for 20%.
Medicare covers 3 hours of initial diabetes nutritional counseling in the first calendar year plus 2 hours follow-up annually. You would be responsible for 20%.
2. Speak with your physician about your need to learn more about managing diabetes. A physician referral is required. Bring a copy of our referral to your physician for him/her to complete and fax to us at 908-237-6922.
3. Call 908-237-6920 for an appointment. It is your responsibility to verify insurance coverage. Contact HMC Patient Accounts (908-788-6194) if you need financial assistance.
4. Bring a list of your medications, your blood sugar diary and blood sugar meter to your appointment.
5. Complete the Diabetes Assessment Form and bring to your appointment.
6. A family member or significant other is invited to participate with the education.
7. If you need to cancel and reschedule the appointment, please give us 24 hour notice so a cancellation fee is not charged.
